LB1025 Histopathologic scarring outcomes of cutaneous surgery in the setting of immunosuppression

Background: Cutaneous wound healing and scarring are poorly understood in the setting of immunosuppressive therapies for anti-rejection post solid organ transplantation. Purpose/Hypothesis: To investigate whether concurrent use agents during cutaneous surgery correlates with less favorable outcomes. Methods: A prospective cohort study at University Iowa studied patients undergoing elliptical excision treatment non-melanoma skin cancers. Clinical assessments (photographs, scar width, Konica Minolta CM-700d Spectrophotometer, POSAS=patients observer assessment scale, bacterial culture swabs) as well tissue samples from excision/scar site collected day 0 14. Tissue were analyzed by an independent, blinded dermatopathologist graded categories of: acute inflammation, chronic fibroblast density, collagen fiber characteristics density blood vessels. Categorical scores summed to produce a total score higher indicating features more consistent increased maturity. Results: Of 23 recruited (all white/Caucasian, mean age 64), 17 completed requirements analysis. 4 subjects receiving therapy transplant time excision. those on immunosuppressants, levels acute/neutrophilic inflammation significantly (MD -1.27, P=0.044, 95%CI -2.50 -0.04) lower 2.75, P= 0.038, 95% CI 0.18 5.32) mature formation Conclusions: Patients show scars decreased histologic markers maturity delayed healing.
